Clexane belongs to the group of "anticoagulants" (medicines that prevent blood clotting).


  • Prophylaxis of venous thromboembolic disease, especially in orthopedic surgery and general surgery
  • Prophylaxis of venous thrombosis-embolic disease in "medical" patients who are bedridden for an acute medical condition such as heart failure, respiratory failure, an episode of acute infection or a rheumatic condition
  • Treatment of deep vein thrombosis with or without pulmonary embolism

Moderate thromboembolic risk
  • One daily injection of 20 mg (0.2 ml)
  • The first injection will be made about two hours before the intervention
  • High risk of thromboembolism
  • 40 mg (0.4 ml), given as a daily injection.
  • The first injection will be given 12 hours before the intervention
  • Recommended daily dose: 40 mg by subcutaneous injection
  • Injection of a dose of 1 mg / kg into the arterial line of the dialysis circuit, at the start of the session
  • If fibrin rings appear: a new injection of 0.5 to 1 mg / kg until the end of dialysis
  • Hemodialysis patients at high hemorrhagic risk: a dose of 0.5 mg / kg (double vascular approach) or 0.75 mg / kg (single vascular approach)


  • either in a single daily subcutaneous injection of 1.5 mg / kg
  • either in two daily subcutaneous injections at a rate of 1 mg / kg every 12 hours
  • Recommended dose: 1 mg / kg every 12 hours subcutaneously in combination with acetylsalicylic acid (100 to 325 mg orally, once a day)
  • Initial dose: 1 IV bolus injection of 30 mg followed immediately by subcutaneous injection of 1 mg / kg
  • Then: 1 subcutaneous injection of 1 mg / kg every 12 hours (maximum 100 mg for each of the first two SC doses, followed by SC doses of 1 mg / kg)

Administration mode

  • The website www.thrombosiscare.be contains a video to explain self-injection to patients
  • The recommended place for injection is lower abdominal fat
  • The injection should be done at least 5 cm from your navel and outward on either side
  • For each injection choose another place of the lower abdomen, alternately on one side then on the other
  • Sit or lie down in a comfortable position and clean the chosen injection site with an alcohol swab
  • Take the syringe and remove the needle cap
  • Do not remove air from the syringe
  • If a drop appears at the end of the needle, remove it by tapping the syringe, with the needle pointing down
  • Make a skin fold between your thumb and forefinger
  • Introduce the needle in the thickness of the fold, perpendicularly and especially over its entire length
  • Gently press the plunger of the syringe; slowly inject the medicine
  • After injection, do not squeeze or massage the skin
  • The package leaflet contains information about IV injection
