Improves blood sugar control:

in monotherapy

• in patients who are insufficiently controlled by diet and exercise alone and for whom metformin is contraindicated or is not tolerated.

in dual oral therapy, in combination with

• metformin, when used as a monotherapy with diet and physical exercise, does not allow adequate glycemic control to be obtained.

• a sulphonylurea, when used as a monotherapy, at the maximum tolerated dose, with diet and physical exercise, does not allow adequate glycemic control to be obtained and when metformin is contraindicated or not is not tolerated.

• a gamma peroxisome proliferation activator receptor agonist (PPARγ) (thiazolidinedione), when it is appropriate and its use in monotherapy with diet and exercise does not allow adequate glycemic control to be obtained .

in oral triple therapy, in combination with

• a sulphonylurea and metformin, when dual therapy with these two drugs with diet and physical exercise does not allow adequate glycemic control to be obtained.

• a PPARγ receptor and metformin agonist, when the PPARγ receptor agonist is appropriate and dual therapy with these two drugs with diet and exercise does not allow adequate glycemic control to be obtained.

Januvia is also indicated in addition to insulin (with or without metformin) when a stable dose of insulin with diet and exercise does not provide adequate control of blood sugar.

Use

Dosage

The dosage is 100 mg sitagliptin once a day. When used in combination with metformin and / or a PPARγ receptor agonist, the dosage of metformin and / or the PPARγ receptor agonist should be maintained, and Januvia administered concomitantly.

When Januvia is used in combination with a sulphonylurea or insulin, a reduction in the dose of the sulphonylurea or insulin may be considered to reduce the risk of hypoglycaemia (see section 4.4).

If a dose of Januvia is missed, the patient should take this dose as soon as he or she realizes it. Do not take a double dose on the same day.
