Indications

  • Substitution of a physiological deficiency of the thyroid hormone in the event of hypothyroidism
  • Suppression of TSH production by the pituitary gland
  • Euthyroid goiter demonstrated by the secretory response to TSH after oral or intravenous administration of TRH
  • Prevention and treatment of goitrogenic complications following the administration of antithyroid drugs in hyperthyroidism
  • Prophylactic treatment after resection of a goiter or during a total thyroidectomy for thyroid carcinoma in order to reduce the risks of recurrences

Use

Substitution

Initial dose: 25 - 50 mcg per day.

Then, this dose should be increased in steps of 25 mcg every 2 to 4 weeks depending on the clinical response.

Maintenance dose: 100 - 200 mcg per day

In the event of coronary insufficiency or cardiac decompensation:

Start treatment with a very low dose (12.5 mcg / day or 25 mcg every 2 days) and increase it very gradually

Reduce the dose and prescribe a betalytic if angina occurs during treatment

Increase maintenance dose by approximately 25% during pregnancy or during estrogen therapy (thyroxine requirements are increased)

For newborns and children with congenital hypothyroidism:

The starting dose is 10 to 15 mcg per kg of body weight per day for the first 3 months

The dose must then be adjusted individually according to clinical conclusions and the levels of thyreotropic and thyroid hormones.

For children with juvenile hypothyroidism:

The recommended starting dose is 12.5 to 50 mcg per day.

This dose should be increased gradually every 2 to 4 weeks depending on clinical findings and the levels of thyreotropic and thyroid hormones.

Maintenance dose is generally 100 to 150 mcg per m² of body surface

Administration mode

In a single dose per day, generally in the morning on an empty stomach, half an hour before the meal

If necessary, the tablets can be dissolved in a little water (10 to 15 ml) and the suspension thus obtained, which must be freshly prepared, must be administered with a supplement of liquid (5 to 10 ml).
